Our clinic now has a new tablet in the reception area. Patients are encouraged to use it when they arrive for medical appointments with their Primary Care Provider. We hope this will make the check-in process quick and easy for our patients and help to alleviate our front desk staff.

In consultation with your primary care provider, patients can now purchase the following vaccines at our clinic:

  • Twinrix (Hepatitis A & B Vaccine)
  • Shingrix (Shingles Vaccine)
  • Gardsil9 (HPV Vaccine)
  • Bexsero (Meningitis B Vaccine)

Preventive health aims to stop disease before it starts – by making your health and well-being a priority.

We ask that all eligible patients to be involved in their own preventative care by participating in breast screening (mammography), cervical screening (PAP test) and colorectal screening (FIT or colonoscopy).  Eligible patients will automatically receive a letter from Ontario Health (Cancer Care Ontario) to:

  • Invite them to participate in screening
  • Remind them when it is time for their next screening test
  • Tell them their screening test results
  • Tell them what to do if they have an abnormal test result.

When patients are screened regularly, we can detect pre-cancerous changes, or cancer at an early stage when there is a better chance of treating it successfully.

Dear Patient,

To begin, I would like to thank you for entrusting me with your health and wellbeing. It is with bittersweet regret that I will be closing my Family Practice effective May 1st, 2025. Please know that I have not come to this decision lightly and appreciate that this news may come as a surprise and will be distressing for many. Please know that I will continue to monitor outstanding tests that have been ordered. If you require a prescription renewal, be sure to schedule an appointment before April 30th, 2025, to complete this.

If searching for a new primary care provider, below are some resources that may help:

  1. Register with Health Care Connect using their website: ca/healthcareconnect and have a nurse find a doctor or a nurse practitioner for you. You may also register over the phone by calling: 1-800-445-1822
  2. Use the College of Physicians and Surgeons of Ontario’s “Find a Doctor” search:

  3. Choose “Advanced Search” to find a doctor near you (by city/town or postal code)
  4. Click on “Additional Search Options” to narrow your search
  5. Contact the doctor to check if they are accepting new patients

 

Please note that at this time, The Nation Health Centre does not have any doctors accepting new patients.

 

Your medical records will remain in the custody of the Nation Health Centre for a period of not less than 10 years (for children: records are retained for 10 years after they turn 18 years of age). To send a copy of your medical records to another provider, the Nation Health Centre requires your written authorization. An administrative fee for this uninsured service will apply.
