Important Announcement: Dr. Samson / Dr. Taheraly Patients

Dear Patient,

As a follow-up to Dr Samson’s departure on May 31st, Nation Health Centre wishes to update you with regards to your medical care.

We are pleased to inform you that Dr. Zainab Taheraly has assumed the care of Dr. Samson’s patients and practice as of June 1st, 2022.   Dr Taheraly graduated from medical school at Paris-Descartes University in Paris, France in 2013. She moved to Ottawa shortly after and completed her family medicine residency at the Montfort Hospital with University of Ottawa. She has been practicing since 2017, serving patients in both English and French in various rural communities east of Ottawa. She has a keen interest in small procedures, joint injections and women’s health. Dr Taheraly also enjoys teaching at the University of Ottawa as a unit leader and community preceptor, helping to train medical students. She is excited to join the Nation Health team and start this new chapter of her career.

For those choosing to stay with Dr Taheraly, you will be required to sign an Enrollment and Consent Form, to maintain your status as patients of Dr. Taheraly and Nation Health Centre. The deadline to return these forms is November 30th, 2022 and they can be found on our website at: Forms.

Please print out, fill in the required fields, sign, and return the completed forms to the clinic before the deadline. If you are unable to print these forms, you may come to the clinic in person on Friday afternoons only between 1-3 PM starting Friday September 16, 2022.

If you would prefer to seek care elsewhere, please be advised that your medical records will remain in the custody of Nation Health Centre for a period of up to 10 years.  To send a copy of your medical records to another provider, Nation Health Centre requires your written authorization.  An administrative fee for this uninsured service will apply.  A Consent to Transfer and Release Personal Health Information form can be found on our website at: Forms.
