YOUR FEEDBACK IS WELCOME
Nation Health strives to provide services in a manner that is accessible to all of our patients, and respects your dignity and independence in accordance with AODA Accessibility Standards for Customer service. Your feedback is important to us. To help us better serve you, we invite you to fill out this form and to give us a written description of the situation that led you to formulate your comments.
All patients are required to provide their health card (OHIP card) each time they visit. If you do not have a valid OHIP card you will be asked to pay for your services when they are provided.
Notify our reception staff when you arrive if you are here to see the nurse for an injection such as a flu shot, allergy shot, TB test or other injection. Since you will need to wait twenty minutes after an injection (in case of an adverse reaction), you will need to register with the reception staff at least 30 minutes before the clinic’s closing time.
Narcotics and other controlled substances will not be prescribed nor renewed for walk-in patients. Please see your family physician.
The way we work together matters. Every person who enters our doors should be treated in a respectful manner. In turn, we expect all of our patients will be respectful towards our staff and providers.
We’ll get back to you within 24 - 48h. We promise, we won’t forget about you!
